**ENTRY REQUIREMENTS** + Graduate of approved School of Nursing. Currently licensed and in good standing with the State of Michigan Department of Licensure and Registration. + Minimum of two years experience in Critical Care Nursing. + Previous experience in outpatient Cardiac Rehab. + Current BCLS and ACLS certification. + Current Clinical Exercise Specialist certification through the American College of Sports Medicine or Cardiovascular Nurse Certification through the American Nurse's Association preferred upon hire, but required to obtain certification for Clinical Exercise Specialist or Cardiovascular Nurse Certification within one year. + Comprehensive knowledge of Cardiac Rehabilitation in the acute and outpatient settings. + Maintains 20 hours of approved continuing education annually (includes mandatory education settings). **ORGANIZATION** Reports to Cardiac Prevention and Rehabilitation Manager. Has a constant working relationship with other hospital staff as: Cardiovascular Service Line Staff, physicians, social workers, and dieticians. Interacts with patients and family members at an inpatient, outpatient and community level. **SPECIFIC DUTIES** + Supports the Mission, Vision and Values of Munson Healthcare. + Embraces and supports the Performance Improvement philosophy of Munson Healthcare. + Promotes personal and patient safety. + Has basic understanding of Relationship-Based Care (RBC) principles, meets expectations outlined in _Commitment To My Co-workers,_ and supports RBC unit action plans. + Uses effective customer service/interpersonal skills at all times. + Demonstrates expertise in Cardiac Rehabilitation Nursing practice, knowledgeable of the health problems of cardiovascular patients and demonstrates ability to help patients achieve and maintain optimal health. + Demonstrates active participation and facilitation of any phase of cardiac rehabilitation. + Effectively initiates, implements and evaluates inpatient education and exercise progression. + Effectively facilitates group education classes and 1:1 sessions with patients and families. + Effectively facilitates and evaluates monitored exercise therapy sessions in Phase II and Phase III. + Effectively participates in patient evaluation, care planning and documentation of patient care. + Effectively participates in the unit's team efforts, work groups and quality improvement teams. + Works with other health professionals and community members toward the education and prevention of heart disease. + Performs all functions and activities with the highest level of professionalism, confidentiality and tact. + Performs other duties and responsibilities as assigned.
